I am at least 18 years of age and will provide government issued ID.
I understand that I will be required to leave a deposit for my tattoo. This deposit is NON-REFUNDABLE and will be forfeited if I cancel less than 48 hours in advance, or do not show up for my appointment.
I understand that there are NO complementary touch-ups for tattoos on the hands or feet, and that only one complementary touch-up appointment is offered per tattoo elsewhere on the body. I understand that if I miss that appointment or cancel less than 48 hours than advance, I forfeit my free touch-up.
