I understand that my child is attending the OHEN Prom at his/her/their own risk and he/she/they, and his/her/their parents, are ultimately responsible for his/her/their own actions and him/her/their self. I, the parent of the attendee, agree to release the organizers from liability for injuries or damage resulting from accident and/or ordinary oversight. Attendee may be asked to leave if behavior is deemed unacceptable by organizers.
