Confirm that I agree: 

1.  To appear and be filmed and/or interviewed for the Series 

2.  That such Interview may be edited at the Producer's sole discretion;

3.  To the Producer incorporating the Interview or any part of the Interview including my image and voice in the Series or any marketing or publicity for the Series;

4.  To the Producer marketing and distributing the Series in any and all formats and all media whether now known or later devised throughout the world in perpetuity;

5.  That I am not entitled to any remuneration for the exercise of the rights granted in this release;

6.  That I will not have any interest in the copyright or other rights in the Series;

7.  That the Producer may use my name, likeness, biographical material and participation in the Series for the sole purpose of promoting the Series;

8.  That the Producer has the right to assign or licence the rights and other benefits granted under this release in whole or in part; and

9.  To release the Producer from any claim or future claim arising from my participation in the Series for this Interview, and I agree that the rights granted by me in this release are binding on me in accordance with the above terms.
