Medical Release/Permiso Medico

I hereby give my permission to the medical personnel selected by Christ Church Cathedral to order x-rays, routine tests and treatment for me or my child(ren), and, in the event I am not able to communicate or cannot be reached in an emergency, I hereby give permission to the physician selected by Christ Church Cathedral to hospitalize, secure proper treatment for, and order injections and/or anesthesia and/or surgery for me or my child(ren) as named prior. I will be fully responsible for any costs of such treatment, even if not covered by insurance.

Yo doy mi permiso para que Christ Church Cathedral pueda solicitar los servicios médicos para el niño mencionado arriba si sufre enfermedad o accidente y el padre / tutor no puede ser contactados. Si no pueden ponerse en contacto las personas mencionadas arriba, autorizo para que ellos tomen las medidas que consideran que es en el mejor interés de mi hijo. As parent or legal guardian I give my permission for my child(ren) to participate in The Joyful Noise Day camp field trips. I understand that there will be adequate adult supervision for these events. All camp staff will have received Safeguarding God’s Children training in advance of camp.
