By signing below, I acknowledge that I understand everything I have read in this application and that the information provided is true and complete. I also understand that any knowingly false or incomplete answers may cause my application to be denied.

I hereby give a representative of Cause for Paws, Inc. permission to contact my landlord, if applicable, and veterinarian to verify any of the information supplied in this application as well as any other pertinent information. I further understand that Cause for Paws, Inc. is considered the guardian of the cat and has the right, in its sole discretion, not to approve this application.

If my application is approved, I acknowledge that I will be the primary caregiver and will be legally responsible for the cat after the adoption has been finalized. I understand that if approved, this application will become a part of the adoption contract and any false information will void the contract.
