Year 6 Sports Ambassador Permission Form
Dear Parents and Carers,

Your child has volunteered to support the EY1 and 2 Multi Sports ECA on a Wednesday after school from 3.15 - 4.15pm.

Your child will be responsible for planning and organising a sports activity for the EY1 and 2 children to participate in.

If you give permission for your child to participate as a sports ambassador for this ECA please complete the permission slip below. By completing this form you agree to your child staying after school until 4.15pm and then to make their own way home.

The first session will be Wednesday 5th February 2020.
