The OMHA Honour Award was established to honour a person who has made an outstanding voluntary contribution to minor hockey for a considerable period of time doing so of his own free will and not as part of a job or position for which they receive or received monetary consideration. Any services performed by recreation directors, arena managers, etc. for which remuneration is received in performance of their duties, shall not count as service in regard to this award.
This award honours a volunteer at the local level and therefore any Officer or Board Member of Hockey Canada, OHA, W.O.A.A., OHF, or other sports governing body shall not be eligible. Any Life Member, Past President, Officer, Board Member or District Convenor of the OMHA shall not be eligible.
However, any person included in the previous paragraph who no longer holds this office or position and continues their volunteer work for minor hockey shall become eligible for nomination after they have not served in that previous office or position for two years.
Any OMHA minor hockey organization in good standing may nominate a person for this award by completing the form below stating the services the nominee has contributed to minor hockey.
The recipient of the OMHA Honour Award receives and retains an engraved silver tray bearing their name.
HOW TO APPLY
1. Nominating Associations are requested to complete the attached application form and submit it, along with a letter of nomination and summary of the details of volunteer contribution to the local association, minor hockey, and community at large summary of the Nominee’s service. Nominations must submitted to the OMHA by 5:00 pm on February 28th, 2020.
2. Nomination must be endorsed by signing officer of the nominating Association Executive.
