NMLB Adoption Application

Our adoption fee is $500.00 unless otherwise noted, payable by cash, bank certified check, money order or credit card ONLY. If you pay with a credit card, a processing fee of $15.00 will be added. This fee is from our credit card payment processing center. We DO NOT accept personal checks.

Any puppy under the age of one year will be required to complete puppy training classes with their adopter. This requries an additional $50 deposit upon adoption. Once a certificate of completion is shown, the deposit will be returned.

In order to be approved to adopt, a good vet reference is a REQUIREMENT. We require all CURRENT cats/dogs in the home to be spayed/neutered, up-to-date on vaccinations and all CURRENT DOGS must be current on monthly heartworm prevention. If you do not meet these requirements, you WILL NOT be approved. No exceptions unless a legitimate medical reason is provided by your vet.

If you have used multiple vets for your pets, please list them ALL. Along with what care each pet received at each vet (spay/neuter, yearly vaccinations, monthly heartworm prevention). If you have used multiple vets and you do not list them, your application CANNOT be processed. Please provide the NAME and PHONE NUMBER of each vet.

If you are a first time pet owner and do not have a vet reference please list N/A or first time owner under vet reference.

***Please call your vet and give them permission to speak to us and notify your personal references we will be reaching out via phone or email (also please check spam/junk folders). Not getting ahold of references is the biggest hold up on getting applications approved immediately. ***
