Phi Upsilon Omicron 2020 Conclave Registration Form

Please use this form to register for Phi U Conclave 2020 which will be held in Columbus, Ohio, Thursday, October 1st - Saturday, October 3rd. You will have the option to preview, print, and submit the form once completed. If you wish to pay by check, please select this payment type and follow the directions per the form below. You will still electronically submit your registration form. If you wish to pay by credit card, a $15.00 non-refundable convenience fee will be added to your total. A green check mark will appear once your registration has been successfully submitted. Registration Information:

• Registration Deadline: Tuesday, September 8, 2020 (If postmarked after this date, add $50 to registration fee.)
• One registration form is required for each Conclave attendee.
• No refunds after September 14, 2020 (Subject to a $25 cancellation fee.)
• Make checks payable to: Phi Upsilon Omicron Gamma Alumni Chapter (Include attendee name(s) on memo line of check.)

Note: ONE collegiate delegate for each chapter receives complimentary registration from the Phi Upsilon Omicron National Office. ONE alumni delegate for each chapter receives a $100 reduced rate registration. All delegates must still complete the registration form and submit electronically. If paying by check, please include a copy of your registration and mail to Joyce Smith. Collegiate delegate does NOT need to include a check for a registration fee; alumni delegate, include a check for the reduced registration fee as indicated below with completed form. All others send payment with registration.
