    COVID-19 PANDEMIC - STUDENT/FAMILY STATEMENT OF OPERATIONS, UNDERSTANDING, AND CONSENT FOR RELIGIOUS EDUCATION

    Section 1: The COVID-19 Pandemic
         The novel coronavirus, COVID-19 (hereinafter “COVID-19”), has been declared a worldwide pandemic and is extremely contagious. As a result, in order to resume in-person religious education at the above-referenced parish, collaborating parishes, or multi-parish community (hereinafter “Parish”), social distancing and other safety
    measures have been enacted at Parish. The Parish has put in place safety measures and standards of behavior to reduce the spread of COVID-19 while attending religious education classes. Even with implementation of safety protocols, Parish cannot guarantee that you, members of your household, or your child(ren) will not become
    infected with COVID-19 and cannot guarantee that attendance at religious education will not increase your risk and/or your child(ren)'s risk of contracting COVID-19.

    Section 2: Direction to Parents and Legal Guardians
         In addition to the safety measures being implemented by Parish, the above-named parents and/or legal guardians (hereinafter “Parents and Legal Guardians”) hereby agree to adopt the following safety measures of their own to help ensure the safety of all religious education students, families, teachers, and staff. Prior to attendance
    of child(ren) at religious education, Parents and Legal Guardians shall take the necessary steps to determine whether their child(ren) are exhibiting symptoms of COVID-19, including, but not limited to, temperature checks for fever (as defined by the CDC as at or above 100.4), review for the presence of any symptoms commonly associated with COVID-19, such as cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, and/or diarrhea. Should your child(ren) exhibit any symptoms of COVID-19, we require that you keep your child(ren) home.

    Section 3. Students Exhibiting COVID-19 Symptoms at Religious Education
         Should a child demonstrate any symptoms of COVID-19 while attending religious education, Parish will contact that child’s Parents and Legal Guardians to require that child be picked up from Parish and taken home in a timely manner.

    Section 4. Known Exposure to COVID-19.
         In the event that a child or a household member living with a child has been knowingly exposed to COVID19, the Parents and Legal Guardians of that child hereby agree to immediately take the following steps:
    1. Contact child’s primary care physician
    2. Notify the local health department.
    3. Follow the local health department’s protocols for quarantine, isolation, and return to work or school.
    4. Notify Parish of the exposure.
    5. Prior to child returning to religious education, Parents and Legal Guardians agree to contact Parish to
    inform Parish of the steps taken to ensure that child is no longer contagious based on the protocols
    and guidelines of the local health department.

    Section 5. Positive COVID-19 Test.
         In the event that a child or a household member living with a child has tested positive for COVID-19, the
    Parents and Legal Guardians of that child hereby agree to immediately take the following steps:
    1. Contact child’s primary care physician
    2. Notify the local health department.
    3. Follow the local health department’s protocols for quarantine, isolation, and return to work or school.
    4. Notify Parish of the exposure.
    5. Prior to child returning to religious education, Parents and Legal Guardians agree to contact Parish to inform Parish of the steps taken to ensure that child is no longer contagious based on the protocols and guidelines of the local health department.
    6. Provide Parish with evidence of child’s negative COVID-19 test result and/or a release from child’s primary care physician.


    Section 6. Acknowledgement of Risk
         By signing this COVID-19 Pandemic - Student/Family Statement of Operations, Understanding, and Consent for Religious Education, I/we acknowledge the contagious nature of COVID-19, that my/our child(ren) and I/we may be exposed to or infected by COVID-19 by attending and/or working and/or volunteering at religious education, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I/we understand that the risk of becoming exposed to or infected by COVID-19 at the above-named Parish may result from the actions, omissions, or negligence of myself/ourselves and others, including, but not limited to Parish employees, volunteers, students, and their families. I/we further agree on behalf of myself/ourselves, my/our child (student) named herein, and my/our heirs, successors, and assigns, to absolutely release, defend, indemnify, and hold harmless the named Parish, teachers, volunteers, and the Archdiocese of Milwaukee, its directors, officers, employees, attorneys, agents, representatives, and insurers/third-party administrators(hereinafter collectively referred to as the "Indemnified Parties") from any and all claims or causes of actions in any way related to COVID-19, brought by any person or entity, including but not limited to, all claims and causes of action based on the alleged negligence of the Indemnified Parties, other third parties, or my own negligence

    The Holy Family Parish photographer or coordinator will be present to take photos or videos of the students for use in program displays, the parish bulletin or website, or other publicity-related publications. These materials will only be used for appropriate purposes and names will not be listed. If you do not wish to allow your child to be photographed or video taped for these publications, you must contact Elementary Formation Coordinator via e-mail prior to the first day of class to develop a plan to keep your child out of photos/videos.

    Non-Parishioner Notice

    If neither parent belongs to Holy Family Catholic Community, you must provide a letter or e-mail from your parish Pastor AND Religious Education Director to Jessica Derks (jessicad@hffdl.org) to verify that our program will meet your formation requirements. It is important that your home parish be notified of your faith formation participation.

    If you choose not provide this letter of approval, your home parish might not allow your child to receive sacraments as faith formation is a requirement during a sacramental year.

    New this Year:

    We will no longer have faith formation every Wednesday night.

    Grades 1-4 will come on the first Wednesday of the month.

    Grades 5-8 will come on the second Wednesday of the month.

    The third Wednesday of the month will be an all school socially distanced Mass with no faith formation that night.

    We did this to minimize contact between the kids. Masks are required for catechists and all students. 

    We are also going to be implementing faith formation for parents this year. While your child is getting catechized, you will be going to Sacred Heart Church at socially distanced pews. We will be giving you resources and ways to bring the faith into the home since we have the kids for limited times. We will also be asking you to work on lesson plans at home. This is to provide a space for all of our parents to learn about the faith and how to pass it on to their children.  We want to provide opportunities for our parents/couples to encounter the sacraments, have great speakers, and be in community with one another. 

    Childcare will be avaliable if needed. 

    If you have children going to both the first Wednesday of the month and the second Wednesday of the month, you are welcome to pick which Wednesday you would like to attend parent formation.

    Parental Involvement

    Welcome to the Holy Family Sacramental Preparation Program! I am so thrilled to be working in conjunction with you to prepare your child for two very important sacraments: First Reconciliation and First Communion. Please remember that while this program is run by the Faith Formation Coordinator, you- the parent- are the initial educator of your child. Through the course of this program you will be given two books- one for Reconciliation and one for Communion. We will do the first chapter of each book together, but the remainder of the course work instruction is the responsibility of the parent.

    The following are excerpts from the Archdiocese of Milwaukee Sacramental Guidelines which relate to regular catechesis/formation, family/parent involvement, and parish involvement.

    - Regular catechesis, provided through the parish school or religious education, together with the formation parents are providing in the home, forms the foundation upon which the immediate catechesis for First Penance/First Eucharist can be built.
    - Parents have the right and responsibility to be intimately involved in preparing their children for First Penance/Eucharist.
    - This parental right and responsibility to direct the religious formation of their children needs to be affirmed and encouraged.

    - A child in the process of Christian Initiation is led gradually, according to personal capacity, to participate fully in the Eucharist. This continuing formation takes place within family and parish community.
    - A child's family and the Christian community's day-by-day living of the Christian life becomes the first step in a child's preparation to receive [these] sacrament[s].

    Required Baptismal Certificate

    The next slide will require you to insert a clear photo of your child's baptismal certificate. You will not be able to continue with the registration form until the baptismal certificate is provided.

    Registration Closes: October 10 (10/10). Please note that your registration is not valid unless a baptismal certificate is provided. If you complete this registration form by 10/10, but do not provide a baptismal certificate, your child is not enrolled in the program.

    If your child was baptized at one of the Holy Family sites and you need a digital copy of the certificate to upload, please contact us at info@hffdl.org with your child's name, month, year, and the location at which they were baptized.

    First Communion Mass Selection

    Mass selections will be filled on a first come- first served basis. 

    Mass selection sign up will be available via the Holy Family website on Wednesday, January 6. 

    Please note that it is best to pick your Mass as close to January 6 as possible as Masses fill quickly.

    Grades 1-8:

    1 Student - $85.00
    2 Students - $170.00

    Grades 9-10:

    1 Student - $100.00
    2 Students - $200.00

    Grades 1-10:

    FAMILY CAP OF 3+ Students - $230.00

    Sacraments (Reconcilaiton/First Communion): 1 Student - $30.00
    Sacraments (Reconciliation/First Communion): 2 students - $60.00
    Sacraments (Reconcilation/First Communion): 3 Students - $90.00 

    *If you have reached the family cap of $230 and have a child in Sacraments, you must still pay the outstanding sacramental fee per student.*

    Financial Assistance

    Financial Aid or a payment plan is available. A separate form must be filled out to explain your family's situation. Please contact scarter@hffdl.org for more inforamtion.

    Please Note: Anyone planning to apply for financial aid during the Yearly Program (Fall of 2020) or Summer Program (Summer 2021) must be a parish member for at least six months prior to applying for financial aid.
