• Interprofessional Student Hotspotting Learning Collaborative, 2020-2021

    You are invited to submit an application to join the Interprofessional Student Hotspotting Learning Collaborative! As a student hotspotter, you will assemble an interprofessional group of students focused on collaboratively gaining insight into the root causes that lead patients to have repeat emergency room visits and hospital admissions, and analyze how this additional utilization might have been avoided. Instead of focusing on specific clinical interventions, you will work with individuals with high utilization of the hospital within your community to understand their unique circumstances and help connect them with community resources that could lead to better health outcomes. Through participation in the six-month learning collaborative, you will receive in-depth training on trauma-informed care, social determinants of health, team building, communication, advocacy and more to help you improve our care delivery systems both now and in the future.


    The Student Hotspotting program is co-hosted by our four Student Hotspotting hubs:

    • Thomas Jefferson University – Philadelphia, PA
    • Samuel Merritt University – Oakland, CA
    • University of Utah – Salt Lake City, UT
    • Southern Illinois University – Springfield, IL

    These four institutions serve as centers of training and mentorship for schools in their region. As such, each team will be paired with one of these four institutions based on indicated preference on this application. Your assigned hub will administer the Student Hotspotting curriculum and support your team through regular check-ins as you launch this program within your own institution.


    Please note that although your indicated preference will be heavily weighted in assigning your placement, we cannot guarantee top preference to all applicants.

  • Program expectations

    Before applying, applicants​ ​must​:

    • Create a team of 4-6 students that represent a MINIMUM of two different professions (3 professions preferred), and one team advisor (see responsibilities below). Greater interprofessional diversity strengthens your application. There are no requirements on professions.
      • It is highly recommended that all team members commit to remaining in the same geography for the duration of the program (Sept-April) to minimize scheduling complications.
    • Identify one project advisor for the team. It is highly recommended that advisors serve as faculty members at an institution involved in the project. Teams are welcome to include additional project advisors at their discretion.
    • Identify a practice or partner site (community organization, hospital, clinic, etc.) that you will work with to identify patients with complex health and social needs.

    During the program, accepted teams will be expected to:

    • Work with the identified clinic/hospital/community partner to select 1-3 patients who frequently visit the ED and/or are frequently admitted to the hospital, then obtain patient consent to work with them for six months through home visits, accompaniment to doctor visits, and facilitating care coordination.
    • Identify potential interventions to propose to the hospital/clinic that might improve the patient’s ability to access needed care and services outside the hospital or emergency room.
    • Assist the patients with appointments and better understanding their healthcare needs and develop an authentic healing relationship with selected patients.
    • Participate in a six-month learning collaborative hosted by the Camden Coalition that includes:
      • Kick-Off meeting, September 2021 at one of the hotspotting hubs across the nation (locations above; events may be held virtually due to safety and travel restrictions for the 2021-2022 cohort).
      • Virtual monthly skills labs, case conferences, mentoring, and a curriculum learning guide that will begin in October 2021.
      • Wrap-Up meeting March/April 2022, at one of the hotspotting hubs across the nation (locations above; events may be held virtually due to safety and travel restrictions for the 2021-2022 cohort).
      • Submit a final project summarizing the project and lessons learned.
    • Approximated time commitment: 3-5 hours per week.

    During the program, project advisors will be expected to:

    • Attend kick-off and wrap-up meetings. 
    • Support students as they move throughout the duration of the program as they work to identify, engage, and support patients with complex health and social needs.
    • Commit to attending one (1) program management call each month with leaders from your assigned hub to help navigate through barriers. 
    • Project advisors should be passionate about complex care, interprofessional teaming, and be willing to help guide students in any institutional barriers that may arise with helping run the project. 
    • Approximated time commitment: 1-3 hours per week. It is recommended that advisors check in with teams on a weekly basis to support them in their navigation of the curriculum and patient engagement.

    Fees:

    • Accepted schools will be required to pay a participation fee of $4,000 to the Camden Coalition to cover programmatic costs for their first team; each additional team will require an extra $2,000.
    • Student teams are required to cover all travel costs for all available team members and at least one project advisor per team to attend the kick-off and wrap-up meetings at their assigned hub (Events may be held virtually due to safety and travel restrictions for the 2021-2022 cohort).
    • Invoices will be sent with acceptances in May 2021 with a 30 day grace period for payment. 

    Note: Applicants should ensure that costs are included in their Universities/Institutions budget prior to submitting an application. The applicant may submit a letter of intent to pay, if they are unable to make payment by the grace period.
