I, {borrowerFull}, have applied for and received a loan for the purpose of making pre-sale improvements to the property located at {listingAddress} (the “Property”). I hereby authorize Notable Finance, LLC (“Notable”), the lender, to designate {representativeName}, whose relationship to me is {relationshipTo}, as my representative (“Representative”).

I authorize the Representative to receive all information related to the loan that is available to me and to take all actions related to the loan on my behalf if otherwise allowable.  These actions include, but are not limited to, requesting disbursement of loan funds to my account, requesting an increase in loan funds, requesting a copy of the loan application and agreement, viewing the loan balance, and canceling the loan.  I give my consent to Notable to respond to these actions as if they were taken by me.

I understand that I will be legally and financially responsible for all actions taken by the Representative.  This includes repayment of loan funds disbursed at the request of the Representative and increases in loan funds requested by the Representative.  I understand and agree that Notable is not responsible for the consequences of any requests or directives by the Representative.  

I also understand that documents related to the loan contain personal information about me and this information may be viewed or disclosed to others by the Representative.  I understand and agree that Notable is not responsible for any disclosure or use of this personal information by the Representative.  

If I wish to revoke this authorization, I understand that the revocation will not take effect until I submit a written revocation request to support@notablefi.com and receive written confirmation of the revocation from Notable.
