Physically apparent disabilities require no additional documentation, although it may be helpful in determining specific accommodations.
All other disabilities require documentation from a physician or counselor/therapist who has a history of treating you for your condition, documentation of a diagnosis, or an EIP from highschool, etc.
Specifically for ESA (emotional support animal) approval, the letter from a physician or counselor/therapist must include something about the length and general nature of the therapeutic relationship, specific symptoms experienced, and how the animal helps alleviate the symptoms. (Letters purchased from an online source where you had only one conversation with the practitioner are not sufficient documentation by themselves.)
For everyone, the section titled "Additional Information" is optional and will not affect the approval of accommodations in any way.
There will also be an opportunity to schedule an appointment at the end. A one-on-one conversation with ADA staff is a part of the approval process for both accommodations and ESAs.
