provide quality therapy services to assist your child in reaching their maximum potential
work with you to attempt to provide services at a time that is best for your family
provide you with education and information regarding your child’s medical diagnosis and treatment plan
provide you with activities and make recommendations to help your child at home
provide you with a monthly schedule for your child’s therapy
inform you of all costs and provide payment options as needed
Family / Caregiver Responsibilities
You are required to be on time for appointments and present 10 minutes before the end of the last treatment session. We understand that your child or another member of your family may get sick and have to miss a scheduled appointment. You must call to cancel the appointment as soon as possible. Please understand that your child’s therapist may also get sick and the appointment will have to be canceled.
If you have a planned vacation or a school function is planned, you must let us know as soon as possible and reschedule the appointment if possible. Any notice of 72 hours or more for schedule doctor visits / vacations, will allow for us to remove the appointment(s) from the schedule with it harming your child's attendance rates. Please be aware that extended time off for any reason no matter how much notice is given, may result in discharge and / or loss of place on the schedule. Appointment slots can not be held for a period of time lasting more that 4 weeks.
If your child’s therapist is taking time off, his/her regular appointment will not be on your schedule. Also, if we are scheduled to be closed, there will be no appointments on your calendar for that day. You are responsible for checking your monthly schedule. If we are required to close due to an unforeseen event or your child’s therapist has to cancel the day of services, you will be contacted via the phone number we have on file. It is your responsibility to ensure we have the correct contact information and that you check your messages.
Progress will be limited if your child does not attend therapy consistently. Limited progress can result in insurance denying reimbursement for therapy services. We are a fee for service business. When individuals miss their appointments we do not get any reimbursement. Most importantly, we cannot help your child if they are not here. If you do not call to cancel an appointment, your child will be discharged after you fail to call to cancel 2 appointments in 6 months. If your child misses 25% of their appointments in 6 months without a doctor's excuse, they will be discharged.
We understand that many children/ families have chronic illness. In that case, you will need to be placed on a flexible schedule, allowing you to schedule the best time for you on a weekly basis. Flexible scheduling is also available for families that have irregular work schedules.
Physician orders for therapy services are required. If your child has Medicaid, a Medicaid Referral is also required. 3 months prior to your child’s orders expiring, we will inform you that new orders must be written. You are responsible for making a doctor’s appointment and requesting updated orders. If new orders are not received when current orders expire your child will not be able to receive services and be placed on hold. If new orders are not received within 2 weeks, he/she will be discharged from service.
If your child has glasses, hearing aids, orthotics, gait trainer, communication device or other medical devices you must bring them to each therapy visit and use them as recommended at home and in the community. If your child uses oxygen, you must have it hooked up correctly and placed on the correct settings.
If your child has had a fever in the last 24 hours, has a contagious illness such as COVID, FLU, pink eye, stomach virus, strep throat, lice, or if anyone living in the home has had any of these illnesses you must call and reschedule the appointment.
It is your responsibility to ensure we have the correct insurance information on file. If your insurance changes and we are not informed you will be responsible for the cost of services received.
Electronics are stimulating and make transitions difficult. Electronics, including phones, must be removed from your child before we come to your car or before bringing them into the office.If your child has their own phone, they must leave it with you or it must remain in their pocket at all times in the clinic. We also ask the child to refrain from bringing in any toys. Electronics and toys are distracting and can prevent progress. We are not responsible for any item left or lost.
Food and drinks should not be brought in unless requested by the therapist.
Therapist will make recommendations and provide home activities. It is important that we work as a team to ensure your child reaches their goals.. Poor progress and failure to comply with recommendations may result in discharge from services.
