Instructions:
1. Your school/officials association may nominate one full member senior high school in each of the seven (7) different high school administrative classifications, as well as one member school in the middle school/junior high school division. Please complete a separate nomination form for each school nominated. A school cannot nominate itself. Be certain to complete the evaluation on page 2 of this form.
2. You are encouraged to meet collectively with the head coaches in all sports in which your school sponsors programs, or the contest officials within your association, to determine which school(s), if any, your school/officials association believes is worthy of this recognition. Rivalry should not be a factor in making this decision. Even the most bitter of rivals are capable of demonstrating exemplary sportsmanship when competing against each other. If an opponent is deserving, nominate the school, no matter how bitter a rivalry it may be.
3. Completion of page 3 will consist of a letter of nomination that should summarize why your school/officials association believes the school being nominated is worthy of being considered for this award by the selection committee.
4. Submit the nomination form(s) via the FHSAA’s online system so that it is received no later than March 25th. Nominations received after this deadline will not be accepted.
