By signing this contract you are agreeing to (please check each box):

1. I consent to Sadie Wild Photography to use the photographs captured from my participation in our agreed Model Portfolio Shoot for portfolio submission, shared with social media pages, submitted for publication/competitions, advertising, marketing, exhibition and/or placed on SWP website to display SWP work with my authorisation and approval.
2. In consideration of my engagement as a model, I grant to Sadie Wild Photography ("Photographer") and his/her legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection or approval. I hereby release Photographer and his/her legal representatives and assigns from all claims and liability relating to said photographs.
