Package 1 Cabin $400 per person

Cabins offer the rustic camp feel for groups of all ages. Most cabins sleep up to 12 people in bunk beds and share community style restrooms just a short walk away. This economical option is great for groups of all sizes or as an add on to other accommodations. You will need to provide your essential needs: 

• Sleeping bags and or linen and blankets
• Pillows
• Personal hygiene products
• Towels

Package 2 Manor House $550 per person

Manor House is a large two story building that can accommodate up to 46 people in 9 bedrooms (6 upstairs, 3 downstairs) with three and a half bathrooms. The kitchen is equipped with a refrigerator, range, oven, microwave, and dishwasher with plenty of plates, glasses, and silverware to accommodate a medium sized group. The living room, with its rock fireplace, is a cozy gathering place providing the perfect atmosphere for small group conferences. A washer and dryer on the main floor complete the amenities.

Package 3 Mountain View Lounge Hotel Rooms $650 per person (for single occupancy additional charges apply)

Set among tall pines, Mountain View Lodge boasts 12 hotel style rooms each accommodating two to four adults. Each room comes complete with a bathroom and vanity. Linens including bedding and towels are provided.

Mountain View Lodge also has a small gym for guests who want to get in a workout between meetings.

In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged,

I, the undersigned, hereby grant to U3Fit and staff & Diamond Arrow Christian Conference Center (DACCC) the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations.  The rights granted herein shall extend in perpetuity, unless revoked in writing to U3Fit staff and DACCC throughtout the world and for any purpose whatsoever, including without limitation for marketing and advertising purpsoes of U3Fit and in any and all media, including without limitation U3Fit & DACCC website.

I acknowledge that U3Fit has no obligation to return any photographs or images to me.  

I hereby Release, waive and forever discharge any and all claims arising out of, or in connection with, such use by U3Fit & DACCC including without limitiation and and all claims for libel or invasion or privacy.  

I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name.  I have read the above release and am fully familiar with the contents thereof.  This release contains the entire agreement between the parties hereto as to the subject matter contained herein.  

I do hereby agree to the services that will be provided by U3Fit's services to me, for my professional health & fitness providers, fitness coaching, personal training, weight loss specialists. 

I acknowledge U3fit will design tailored excercise programs for the participants that reflects the participants objectives, fitness levels and experience.  I hereby affirm that I am in good physical condition and do not suffer from any disability that would prevent or limit my participation in this program.

I understand that I must be cleared of any risk factors associated with physical acitivity before I participate and services offered.  If risks are identified, I understand that I am required to provide the staff of U3fit with an official physician's statement indicating that I have been cleared to engage in a physical activity program.

I understand, it is my personal responsibility to consult with my physician prior to my participating in any physical fitness program, and more specifically, the U3fit Revivie Retreat.

I understand, excercise involves certain risks that my include but not limited to serious neck and spinal injuries resulting in complete or partial paralysis, heart attack, stroke or even death.  Also injuries could occur to bones, joints or muscles, slips, falls and unintended loss of balance could result in muscular neurological, orthopedic or other bodily injury.  

I understand that part of the risk involved may be in undertaking any activity or program is relative to my own state of fitness or health physical, mental, or emotional and to the awareness, care and skill which I conduct myself in the activity or program.  

I understand that I have the complete right to stop or decrease exercise at any time furing a session and that it is my obligation to inform any U3fit team memer of any symptoms such as fatigue, shortness of breath or chest discomfort.  

I recognize that the U3fit team requires physical exertion that my be strenuous and may cause severe physical pain and injury.  I hereby agree to not hold any U3fit team member or Diamond Arrow Christian Conference Center responsible from any and all injuries accidents, costs, losses, causes of action, claims, damages and/or liability that my result from the participation in the U3Fit Revive Retreat.  I understand that participation is strickly voluntary and I am willing to assume any and all potential risks involved in participation in the U3fit Revive Retreat activities provided.

I DECLARE THAT I HAVE READ, UNDERSTAND AND AGREE TO THE CONTENTS OF THIS U3FIT REVIVE RETREAT AGREEMENT IN ITS ENTIRETY. I UNDERSTAND THAT THE ASSUMPTION OF RISK WAIVER OF LIABILITY AND ACTIVITY POLICIES AND PROCEDURES ARE INTENDED TO BE AS A BROAD AND INCLUSIVE AS PERMITTED BY THE STATE OF CALIFRNIA AND AGREE THAT IF ANY PORTION IS HELD INVALID, THE REMAINDER WILL CONTINUE IN FULL FORCE AND EFFECT.
