In connection with the subject identified above, I give permission, understand and acknowledge that the participant's likeness/image or depiction has been digitally recorded (“Digital Image”) by or on behalf of Natural Pathways Learning Centre (NPLC) for use by NPLC in promotionaland other activities as described below. “Digital Image” includes video recording, photographs, and any image technology in any media.I hereby authorize, give my full consent and grant all rights to NPLC and its assigns, successors, officers, directors, employees, agents, officials, servants, and representatives, to reproduce, display, and disseminate such photographs and images of the participant, in whole or in part, oraltered in character form and to use the participant's name and likeness/image and the Digital Image in all forms and medias by NPLC for educational purposes or publicity, advertising, sale and promotion including but not limited to use in posters, presentations, programs, publications, flyers, brochures, web site, digital media, videos, displays, and general NPLC promotion, or other lawful purpose throughout the world. I hereby grant permission to NPLC to edit, crop, or retouch such photographs or digital images, and waive any right to inspect the finalphotographs or images. I hereby assign all rights of copyright without compensation or report to me in the Digital Image to NPLC and hereby waive all rights in and to the Digital Image including any moral rights I may have in the participant's likeness/image and/or the Digital Image.I expressly release NPLC and its, assigns, successors, officers, directors, employees, agents, officials, servants, and representatives, from any and all claims and demands arising out of or in connection with any such use of the Digital Image, including without limitation, any an all claims for invasion of privacy, infringement of my right of publicity, defamation (including libel and slander), or any other cause of action arising out of the production, distribution, modification, broadcast, exhibition or other exploitation of the Digital Image.I acknowledge that I have read this entire document, I give this release voluntarily, with full understanding of its contents and with the knowledge that NPLC is relying on the foregoing. I hereby acknowledge and confirm that I have authority to execute this release on behalf of the minor participant.
Participating at Natural Pathways Forest and Nature School, an outdoor, nature-based program, includes inherent and foreseeable risks of harm. You are required to accept these and other risks as a condition of your participation. While the variety of risks is more than can be listed here, and keeping in mind that Natural Pathways Forest and Nature School educators are trained, experienced and make a strong effort in supporting participants to stay safe while learning and playing in nature, the following sections cover the main areas of risk that you are required to acknowledge and sign your name indicating permission to participate for yourself as a adult participant.The Risk Assessment Summary is intended to enable you to better understand and accept the various risks involved at Natural Pathways Forest and Nature School. It is your responsibility to weigh and evaluate the risks. You are under no obligation to participate in all activities at Natural Pathways Forest and Nature School and you may choose not to participate at any time after you sign this form.Section 1: Although we do everything we can to prevent injury, including completion of all Forest School Canada (FSC) risk assessments, Natural Pathways Forest and Nature School is an outdoor program and therefore there is an inherent level of risk. We encourage participants to take supported risks in order to learn how to navigate and manage risk in a responsible manner. Risk/benefit assessments are completed on every activity, every day and the risk of outdoor activity is reviewed regularly. We have a low student:teacher ratio (6:1), plus volunteers.
Section 2: You will be taught fire safety rules. Participants will not be permitted to participate in fire preparations unless the safety procedures are followed. The teachers reserve the right to exclude participants whom they deem are not well prepared. Before participants actually have a fire or use tools, time is spent in preparation. Before any class is allowed to have a fire, safety procedures around fire are taught and practised.
Section 3: You may be using tools such as hammers, knives and hand drills after being well prepared and only when closely supervised. Participants are taught correct tool use and before participants use tools, time is spent in preparation.
In case of an injury to my child or myself while in the care, custody, or control of Natural Pathways Forest and Nature School, I will hereby waive all claims against NPLC, its Directors, Facilitators, Teachers, Volunteers, and Members of all responsibility for any injury, loss, or damage which I or my child might sustain while participating in any NPLC activity.
