Acknowledgement

I understand that all sports carry a risk of injury and that Taekwondo4Fitness is not responsible for damages or loss suffered from participating in these activities.

Taekwondo4Fitness recognises the need to ensure the welfare & safety of all young people in Taekwondo. It will follow the guidance of the child protection policy and implementation procedures.

Following the child protection policy, we will not permit photographs, videos, or other images of young people to be taken without the child's written consent or the parent if the child is under 16.

Taekwondo4Fitness will take all necessary steps to ensure the photo and video recordings are used solely for the purposes they are intended for, which is the promotion and celebration of the activities of Taekwondo4Fitness.

If you become aware that these images are being misused, inform us immediately. This form will be stored safely for personal records and not shared with the organisations mentioned above.

Taekwondo4Fitness does not charge a membership or joining fee but a small handling fee of £5 for the registration and renewal.

Taekwondo4Fitness will give 30 days' written notice of any fee increase.

Medical Authorisation

In the event of an injury or medical emergency at the venue, I allow Taekwondo4Fitness to provide first aid, arrange transport to a medical facility, and authorise emergency treatment as recommended by a doctor for the athlete named above.

I understand that all reasonable efforts will be made to contact the emergency contact if an injury or medical emergency occurs.
