By submitting this application, you are asserting that child named will be committed to attending events and meetings, and participating in all activities. You agree that the child named will be respectful and exhibit good behavior at all times. You understand that failure to comply to these terms will result in the child being dismissed immediately from the program.
Participant Release: I authorize Teens in the D Broadcast Media Group, and those acting pursuant to its authority, to (1) Record my participation and appearance on videotape, audiotape, film, photograph or any other medium; (2) Use my name, likeness, voice and/or biographical material in connection thereto; and (3) Exhibit or distribute such material in whole or in part without restrictions or limitation for any publicity or promotional purpose which Teens in the D, and those acting pursuant to its authority, deem appropriate
