Things to Consider Before Application

*Please make sure you are committed to wanting to adopt before submission. FBS is ALL VOLUNTEER based and we all have "regular" jobs and families. Processing applications is just one of many tasks we have to make sure we are able to save lives.
*Incomplete/incorrect applications will not be processed. Common mistakes are missing or incorrect phone numbers, email addresses, and incorrect vet information.
*Please know we cannot approve everyone for the same dog or puppy. If you're not chosen it doesn't mean you are unfit for adoption. Please consider listing more than one animal you maybe interested in on your application or let us know if another dog in the future fits your lifestyle.
*Please feel free to stay in contact with us via email. It is just two of us that run daily operations and sometimes we need a reminder! We appreciate your patience!
*Meet and greets are primarily scheduled on weekends! FBS is run by dedicated volunteers who have full time jobs as well. We do not travel or meet for adoptions.
*If a meet and greet is scheduled and the animal is a fit, we expect the adoption to be finalized at that time and they will be able to leave with you. Plan ahead please.
*We do not ship, transport, or do Zoom/Skype calls for you to meet the animals.

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM (required) Read and submit with your adoption application. This form is used in the event a meet and greet is scheduled to protect the rescue and their board members/volunteers from accidents that can occur when dealing with animals (scratches, etc).I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.I certify that there are no health-related reasons or problems which preclude my participation in this activity.I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: For Belle’s Sake Rescue and Rehabilitation, (FBS) and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers;(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.I acknowledge that FBS and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.I acknowledge that this activity may involve risks.
