Paper exams:  Please allow at least two weeks prior to date of your first scheduled exam.  In order to complete this Exam Order form you will require a the mailing address for your proctor and a copy of the signed proctor security agreement form.Proctor Security Agreement which you can find here.

Online exams: Once you have completed the order form you will receive an invoice for the exam fees via email.  After you have paid the exam fees you will receive an invitation to schedule your exam through the online proctoring service (ProctorU).  If you don't receive an invitation within 24 hrs of payment of the exam fees, contact C-NRPP so that the team can resend the invitation. 

Additional information:

• This order form is for a single exam, however it is possible to order both measurement and mitigation exams at the same time, provided that both exams are written within five calendar days of one another.
• Unused exams must be returned in good condition and with seals intact (eg. date or time receipt stamps, scribbling, etc. will render a booklet unusable and result in an exam fee.)
• Payment must be made before the exams will be shipped or online arrangements confirmed.  An invoice will be emailed once the order form has been received and the invoice can be paid online with credit card.

Exam Costs: 

• $200 for the first (paper) Measurement and/or Mitigation Exam; $55 for additional exams (in the case of multiple students writing the exam at the same time, with the same proctor), rewrites are $100 per paper exam
• $175 for the Online Measurement and/or Mitigation Exam
• $125 for the first (paper) CRNCH Exam; $55 for additional exams (in the case of multiple students writing the exam at the same time, with the same proctor)
• $100 for the Online CRNCH Exam
• $100 for any exam re-write
