Update (April 2025):

If you plan to perform a song that was listed on an HAR-082 Song Registration Form that you submitted on or after January 2025, you do not need to resubmit that song on an HAR-082. You must list any other songs that you plan to perform on an HAR-082 Song Registration Form and submit the form at least 30 days before the contest in which you plan to perform those songs.

Contestants are required to upload a copy of the first page of each song, showing title, arrangement, and copyright information. Files may be either pdf or image format (jpg, png, etc.), but they must be clearly legible. Before getting started on this form, please complete the following steps for each song:

1. Scan or take a photo of the first page of each song. 
2. Save the scan or image to your device in a place you can easily find. 

Tips:

• iPhone users: you can scan documents in the Notes app
• Android users: you can scan documents in the Google Drive app
• If you are using a phone to take the photo and a computer to complete the form, email the photo to yourself so you can access it on your computer.
• If you already have a digital copy of the sheet music, you may upload the entire song.

Video Recording Authorization Statement

We give Harmony, Inc. permission to record our performance at the Harmony, Inc. Area/International Convention and Contests and authorize Harmony, Inc. to use any portion of its performances as they deem proper.  This may include, but is not limited to, educational materials for internal use and/or commercial recording(s) produced and marketed by Harmony, Inc. 

If YES, and a copy of the recording is purchased by us, we agree that no copies of the recording will be made by us, nor will we have copies made by anyone without obtaining any and all applicable legal licensing permission for making copies of the recording.

Reminder that NO contest videos including any performance on the venue stage, i.e. contest, showcase, mic testing, Saturday night, etc. may be posted on social media before Harmony, Inc. has made the posting as this violates copyright.

Song Information

Please list songs in order of most likely to sing so that clearances can be prioritized as such. You may submit as many songs as necessary if you have not yet finalized your selection(s).

Note: You may submit more than one HAR-082 form.  Please be sure there are no duplicates.
