I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Fuzzy Texan animal fostered by me.

I understand that by signing this contract that this does not guarantee approval to adopt and should I intend to apply for adoption that I must follow the outlined adoption proces set forth by Fuzzy Texan. I understand that my cooperation and behavior during my foster period will reflect directly on my ability to adopt if I apply and I understand that the rescue will take this into account.

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronic Fuzzy Texan and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog or cat (s) actions at all times, and release Fuzzy Texan from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have Fuzzy Texan complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the dog or cat(s) anymore that I will return the dog or cat(s) to Fuzzy Texan and try to give Fuzzy Texan a 2 week period to try and find another suitable foster for the dog or cat(s).

Fuzzy Texan works hard to ensure all of our animals are healthy. With the assistance of our foster parents, animals in our program will be flea treated, dewormed, vaccinated, spayed/neutered, and seen by our partner veterinarian as necessary. I agree to provide my foster animal with veterinary care only as authorized by FTAR. I will not arrange or pay for any elective veterinary care for my foster animal without the express consent of an authorized FTAR representative, in the event that I do, I will be 100% liable for all related costs.

I understand this is a rescue animal that has come to us from less than ideal conditions and we cannot guarantee the health of the cat/kitten. I understand that FTAR provides no guarantee as to the health of my foster animal and that my foster animal may have medical needs, socialization problems, and may not be housebroken.

It is the foster’s responsibility to quarantine the animal for 14 days (unless they have previously been quarantined at the FTAR approved clinic or a neonatal foster's home) before exposing them to any household pets. Your pets will need to be healthy and up to date on vaccinations in order to foster with Fuzzy Texan.

I understand that I will need to return with the foster animal to the FTAR approved clinic for your area for all wellness (unless arrangements have been made beforehand otherwise). Failure to have my foster(s) vaccinated at the appropriate times will be cause for me being terminated as a FTAR foster.

I agree that I am fostering this animal for FTAR, and that I do not have any right of ownership over my foster animal. I further agree that FTAR 's rights in and to my foster animal are superior to mine.

I agree to provide a FTAR representative access to my home and property to check on my foster animal, at any time that I am in possession of my foster animal. I agree to immediately return any foster animal in my care to FTAR at the request of its authorized representative at any time and for any reason.

If FTAR is forced to undertake legal action to enforce this provision of the agreement, I agree to indemnify FTAR for all court costs and attorneys fees connected with such an action.

I will not transfer possession or custody of my foster animal to any other person at any time, except for temporary, short-term possession for the purpose of vet care, grooming, etc.

I agree that I must submit the animal as requested to Fuzzy Texan when requested as they are the owner of foster animal.

By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully and AGREE to all the terms. I have read this application and agreement in it’s entirety and I agree that all statements and agreements contained in this document are made by me and are truthful, under penalty of perjury under the laws in the State of Texas.
