Alderwood Soccer presented by the CR Soccer organization is a youth soccer program located in the Alderwood community for children 5 - 9 years old. The program welcomes children of all skill levels. Alderwood Soccer is ran by qualified coaches from the CR Soccer Academy (www.crsocceracademy.ca).

1. 2024 Winter Season Weekly Program: THURSDAY'S: 6:00 PM - 6:55 PM (5 - 9 years old)

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by 'Alderwood Soccer' presented by the CR Soccer INC. Organization during the selected time period at Lanor Public School. In exchange for the acceptance of said child’s candidacy by CR Soccer INC., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless CR Soccer INC. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from CR Soccer training (Alderwood Soccer).

In case of injury to said child, I hereby waive all claims against 'Alderwood Soccer' an afiliate of the CR Soccer INC. organization, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to CR Soccer INC. (Alderwood Soccer) and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Photography & Video Release

I hearby grant CR Soccer INC, permission the right to photograph my child's image, likeness and sound of their voice as recorded on audio or video tape. I understand that my childs image may be edited, copied, exhibited, published or distributed on social media channels such as Instagram, Facebook, Twitter & Alderwood Soccer or CR Soccer website (www.alderwoodsoccer.com, www.crsocceracademy.ca)

Photographic, audio or video recordings may be used for the following purposes:

Social Media
Educacational Settings 

By signing this release, I understand this permission signifies that photographic or video recordings of my child may be electronically displayed via the Internet or in the public educational setting. 

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

This release applies to photographic, audio or video recordings collected, video clips and image, online, and Social Media channels as part of the sessions listed on this document only. I agree and acknowledge that photography and/or video can be used even after my child has left the soccer program. 

Additionally, I waive any right to royalties or other compensation arising or related to the use of my childs image or recording.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
