Release of Liability Waiver

SOUTH LYON AREA RECREATION AUTHORITY ADULT LEAGUE WAIVER AND RELEASE OF CLAIMS:

 For and in consideration for my participation in the South Lyon Recreation Leagues (hereinafter the “league”), I hereby, for myself, my heirs, executors, successors and administrators assume any and all risks associated with my participation in the LEAGUE, and release and waive any and all rights and claims that I may now, or in the future, have against the SOUTH LYON AREA RECREATION AUTHORITY, and all parties involved, employees, directors, officers, and affiliates of the LEAGUE, arising out of my participation in the LEAGUE, or any related activities. I hereby fully release and discharge SOUTH LYON AREA RECREATION AUTHORITY, and all parties involved, employees, directors, officers, and affiliates from any and all claims from damage or loss, including, but not limited to, any alleged negligence or unreasonably dangerous condition, which I may have or which may accrue to me from my participation in the LEAGUE. I further agree to indemnify and hold harmless and defend the SOUTH LYON AREA RECREATION AUTHORITY, and all parties involved, employees, directors, officers and affiliates from any and all claims resulting from injuries, damages and losses sustained by me and arising out of, connected with or in any way associated with the activities of the LEAGUE. I acknowledge that SOUTH LYON AREA RECREATION AUTHORITY is not responsible for any inclement weather or acts of God which may affect the LEAGUE and release and waive all claims against SOUTH LYON RECREATION, and all parties involved, directors, employees, officers and affiliates for damage or loss that may arise therefrom. I HAVE READ AND FULLY UNDERSTAND THE ABOVE WAIVER AND RELEASE OF ALL CLAIMS AND UNDERSTAND THAT BY SIGNING THIS DOCUMENT I HAVE AGREED TO WAIVE AND RELEASE ANY AND ALL CLAIMS THAT I MAY HAVE AGAINST SOUTH LYON AREA RECREATION AUTHORITY, AND ALL PARTIES INVOLVED, EMPLOYEES, DIRECTORS, OFFICERS AND AFFILIATES ARISING OUT OF MY PARTICIPATION IN THE LEAGUES.

COVID-19 ADDENDUM

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19: The corona virus or COVID-19 is extremely contagious and thought to have caused a worldwide pandemic. It can readily be spread by person-to-person contact or contact with infected surfaces or areas. In response, Federal, State, and local governments, and governmental health agencies have recommend, or even required people engage in social distancing protocols as well as have prohibited the congregation of people in groups of various sizes. SLARA has put in place preventative measures to attempt to limit the spread of COVID-19; however, SLARA cannot guarantee that you or your family members will not become infected by COVID-19. Participation in a SLARA event can increase your risk of being exposed to the COVID-19 virus. I acknowledge the extremely contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to, or infected by COVID-19 through my attending or participation at a SLARA event and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of becoming exposed to or infected by COVID-19 at any SLARA event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, SLARA, its directors, organizers, coaches, sponsors, manager, attorneys, employees, or any other appointed supervisor. I voluntarily agree to assume all risk sand accept as my sole responsibility any injury to myself (including, but not limited to, personal injury, disability, and/or death),illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation or attendance at any SLARA event. On my behalf, and on behalf of my heirs, I hereby release, covenant not to sue, discharge, and hold harmless SLAR, its directors, organizers, coaches, sponsors, manager, attorneys, employees, or any other appointed supervisor from such, including all liabilities, actions, damages, or expenses of any type that may arise out of, or relating thereto attendance and participation. I understand and agree that this release includes any claims regarding the actions, omissions, or negligence of SLARA, its directors, organizers, coaches, sponsors, manager, attorneys, employees, or any other appointed supervisor, whether a COVID-19 infection occurs before, during, or after participation or attendance at any SLARA event.
