Please NOTE:  We are asking you to REVIEW each potential survey question. You are not taking the survey.

We would like to know if the question is a "Yes" (meaning it looks good. Use in Survey as is),"No" (Remove from Survey), or "Modify" (meaning use the question but am providing suggestions for changing the question).  We've provided a space for your comments (for modifications, etc).
