Form Designer Note (MMC-B 5/8/24):

If the Ministry is "Membership" or "Baptism" and the email recipients should happen to change, please ensure that the email recipients for the condition associated with Send Notification 2 & 3 and the Notification 2 & 3 are the same. 

This looks to be an issue with Jotform that the recipients don't stay in sync when an update is made to the list of recipients.

If Ministry=Membership, this form also auto-notifies the Membership Clerk to assign new members to a shepherd that have completed orientation and are receiving the RHOF.
