Football Academy Trial Registration Forms

FOOTBALL ACADEMY TRIAL REGISTRATION
Welcome to NLP Football Academy. Please note this form is for TRIALIST registrations only. The trial is free and allows players to attend sessions for the specified day only. Once submitted we will add you to the day register for the specified location. This form must be completed and submitted BEFORE and not DURING the trial to be added to the register or they will not be allowed to participate. Please note trial registrations cannot be transfered to a later date if you are unable to attend on your registered date. Following the trial, if you are successful you will be required to complete and submit the full registration and consent form with termly payment which will cover your participation in games, training, tournaments, and events. The full registration must be completed before attending the first team traiing session. If you fail to register in time the coaches are not permitted to allow you to take part.
TRAINING GROUND: Please note we utilise grass, Astroturf, and indoor venues for training. Full address will be sent by whats-app group admins upon your registration. We also ask that parents respect the coaches by keeping a safe distance outside of the marked pitches, do not run onto training sessions for any reason, or interfere with coaching sessions and matches.
KIT: All players must wear suitable training kit including top, shorts, football socks, shin pads, and moulds and astro turf boots. Each player must also bring their own bevearge i.e. water, and no fizzy drinks.
WHATS-APP GROUPS: All information will be communicated through whats-app groups. Only group administrators can post messages in the groups. If you need to communicate any feedback, or confirm any information, please message group administrators or coaches individually. We also post weekly updates in each group. Please note the whats-app groups are exclusive and if you fail to attend any sessions for more than a month without notifying us you will be removed from the group permanently.
DEADLINES: Deadlines are set in advance for participation in tournaments and events. Please ensure they are met on time. If a full squad is not confirmed by set deadlines we will cancel the opportunity.
SAFEGUARDING: Any form of physical, verbal abuse or bullying towards staff, parents, and players will not be tolerated. If you are a parent or a child and believe you have witnessed an incident, due to sensitivity please do not communicate or address any safeguarding issues publicly, and report it to one of the coaches right away who will investigate promptly. Any deliberate abuse or bullying will be dealt with internally. Please remember as a member of the academy we all have a duty of care to uphold to each other.
FEEDBACK: We also have feedback forms which will be sent to all whats-app groups. We appreciate your constructive feedback which will be shared between the staff. Feedback forms are accepted every 8 weeks and we will do our best to respond to these directly.
TRIALISTS/REFERRALS: Trialist’s have a month to settle in to the academy and register using the same process as current members. If they are successful during or after the month they will be invited to join permanently. If you plan to refer any new players please make sure they register through the proper channels with us before they attend. If they are not on the coaches register by their first session they will be not be able to participate.
MEMBERSHIPS: Memberships per player will vary and be discounted for longer and multiple registrations i.e yearly, quarterly, monthly, and weekly and be debited per specified period and stopped at the end of the last operating month. These can cover for sessions and games during that month for specified days. We will provide any updates in advance for separate costs for tournaments or academy games which will be collected via online payment. All payments are non-refundable if players do not attend, and they cannot be rolled over to following months as they will go to all costs committed to that month. If we should cancel any dates for any reason we will re-arrange at no extra cost. Please note there is a 0.95 p charge on the memberships to cover transaction charges. Those on memberships can also attend our other academies if you miss your main session day (i.e. if you miss Saturday at Stratford you can attend to following Thursday at Walthamstow to cover) providing administrators are notified in advance via WhatsApp or email, and those not on registered memberships can attend those academies providing they register in advance by completing this form.
PAYMENTS: We do not accept cash in hand. To set up payment via direct debit you will need to fill in the registration form and direct debit setup link to add your bank details into. Your child will then be added to the register which will be provided to coaches before each session and game. Coaches will reject any cash, and have the right to turn any players away who are not on the register before the first session of the month and not during.

Please note only one free trial is permitted per player - if you attempt to book a second free trial after completing one, coaches will be notified, your registration will be blocked and you will no longer be able to attend.
