ARRIVAL Please aim to arrive 5-10 minutes before your scheduled appointment time. If you arrive after your scheduled appointment time, it may not be possible to extend the time available for your booked service; if your service is shortened due to your late arrival, you will still be charged the full cost of the service.
CHANGING YOUR APPOINTMENT
A minimum of 24 hours notice is required to reschedule/cancel a booked
appointment without penalty.
CANCELLATION/NO SHOW POLICY Less than 24 hr notice, it will result in a charge equal to 50% of the reserved service amount. No shows will be charged 100% of the reserved service amount automatically.
SICKNESS OR FAMILY EMERGENCY
If you, or another person in your household, has an infectious or contagious illness, please contact us as soon as possible to reschedule your appointment for a later date. For your safety and that of staff and other clients, please do not come to your appointments sick. If it is assumed you are currently sick, your appointment may be cut short or cancelled and rescheduled for when you are healthy again. A one-time allowance of last minute cancellation or reschedule will be permitted for sickness or family emergency. After that, the cancellation and no show policy is in effect.
All services are non-refundable but I will be more than happy to schedule a corrective service free of charge. The service must be a mistake from the service provider...not something you might have changed your mind (after the service has been performed).
I give permission for my photo to be taken and my images used or shared on social media.
I hereby release Ashley Johnson Studio, LLC (Ashley Johnson) from any and all
liability arising from all services rendered.
I acknowledge that the hair extension service is final after the application. Any
changes to the style after application will be charged accordingly and at an additional cost.
I acknowledge that hair extensions are more sensitive and differ from intact human hair. I have been informed of the daily maintenance procedure and agree to follow the guidelines to keep my extensions in the best condition possible. I am fully aware and responsible that if I brush my hair too hard, if the extensions are pulled too aggressively, if I choose to use improper washing technique, etc that any damage to the hair application or hair extension themselves is my responsibility.
I acknowledge that I have inspected and approved the hair extensions that are to be installed in my hair. In the event that I decide not to keep the hair extensions at a later date, I am still fully responsible for the total payment of all services rendered.
I understand the explanation of the process I am choosing for my hair. I am aware that with proper care on my part, extensions should remain in my hair for at least 6 weeks. I understand that if an allergic reaction occurs, I will not hold my technician or salon at fault. The charge for the removal of hair extensions is not included in the original fee.
I understand my deposit is non-refundable, and if I cancel my appointment I forfeit my deposit.
