We have updated the Screening Questionnaire to come in line with the advice of MOH as of 19 Mar 2020.

All visitors to TRAC Office should complete this screening questionnaire before coming to TRAC Office in order not to waste a trip.

If there is any “Yes” answers to any of the above questions, you should be asked to see a GP and then rest at home.

The question on WHERE the person has travelled to is for contact tracing purposes and there are no countries exempt from this.

Healthcare workers who work in the front lines and in treatment centres such as NCID are NOT considered ‘close contacts’ as they would have been adequately protected whilst at work treating COVID-19 patients. They may enter the office if they do not have respiratory symptoms.
