Loaner Agreement
Magnus Magnetica endeavors to continually improve the service and support we provide our customers.

Access to, and related costs of a Loaner, will be determined by the original purchase date of the PEMF device being serviced.
- When an “In-Warranty” repair (start date is determined by original delivery date of device) is required:
  1. Within the 6 months of ownership there is no cost to customer, except shipping to/from of Loaner..
  2. After 6 months and prior to the 2nd year of ownership, a Loaner will be supplied at a flat rate of $15 per day for a maximum of 14 days (start date is determined by delivery date of Loaner)
  3. In 2nd and 3rd year of ownership, a Loaner will be supplied at a flat rate of $25.00 per day for a maximum of 14 days (start date is determined by delivery date of Loaner)
  4. Repair delays, exceeding 3 days after the device has been received by Factory, will extend the 14 day limit to 21 days.
- When an “Out of Factory Warranty” Repair is required a Loaner is supplied at a flat rate of $35 per day.
  1. The start date is determined by delivery date of Loaner
  2. The termination date is determined by "Scan in date" at Fedex, when the Loaner is returned
- Loaners are shipped based upon customer selected Fedex Services. Please select from the following:

- Upon receipt of this Signed Agreement, you will be taken to another form to fill out banking information. You will be authorizing Magnus Magnetica, LLC to withdraw, via ACH, the $1250.00 Refundable Security Deposit .
  
  Upon receipt of your Authorization the next section of this form will be completed by Magnus Magnetica and sent back to you prior to shipment of your PEMF Loaner Device.
  
  Upon return of the Loaner, the Security Deposit , minus any applicabe fees will be refunded.
