With Covid-19 spreading across the USA as it is in the rest of the world, VerTek LLC is taking the lead on being pro-active; to protect our team members with a safety and response plan.
This form is part of the response plan. It's for the use by managers, supervisors, and contractors to manage the reporting of team members (in-house and contractors) who appear sick, are sent home, call in sick, are hospitalized, or diagnosed with an active Covid-19 infection.
If an employee is Covid Positive based on a VIRAL TEST, they must contact their doctor and be sent home to quarantine.
Do you have to do contact tracing? No! The place that sent the positive results does that. But, if you know who they came in contact with, please list them on the form. Let HR deal with contacting the customers.
Due to Privacy Laws, do not disclose the name of the person who tested positive for COVID infection.
NOTE: If a person tests positive for an ANTIBODY TEST, contact your local health department for guidance.
