Electronic Invoice Information

You will receive an electronic invoice via your email address for an amount based on the total number of candidates indicated on this application that you plan to enroll. In the event you are not an Authorized Official, a copy of the invoice will also be sent to the email address of the Authorized Official provided.

You may enroll up to 10 Candidates per enrollment application. The fee per candidate is (USD) $15,000.

Be sure to provide us with a Purchase Order Number and Electronic Billing Email for billing purposes in the spaces provided on this application if your organization requires such information for accounts payable purposes.

Payment is due immediately upon receipt of electronic invoice to your email or electronic billing email and must be paid online in accordance with the terms of our electronic invoice. WE DO NOT ACCEPT CHECK PAYMENTS BY POSTAL MAIL, COURIER, OR CARRIER DELIVERY.
