At Discovery Camp, Snow Valley we want to ensure that we have the best and most up-to-date health and wellness information concerning your camper. Even if your camper is perfectly healthy, we need all health information on file, so that we can respond to any immediate need without hesitation, should an accident or emergency occur.

This form is only required to be completed once per camper, even if registered for multiple weeks. If any information contained in this form changes between completion and the start of camp or throughout camp, please update our Camp Director via email at summercamp@skisnowvalley.com.

This information is confidential and will be shared amongst health care providers as needed, i.e.. Walk in Clinic or Emergency Health Care Providers. This information will only be shared with Camp staff on a need-to-know basis to ensure the physical and mental health of your child.

During their time at Discovery Camp, your child may be photographed or videotaped by, or on behalf of, Snow Valley. By completing your child’s camp registration and wellness form and completing the acknowledgment below, you grant Snow Valley permission to use these images or recordings for promotional, marketing, and social media purposes.

You acknowledge and agree that any such images or recordings may be used without further compensation or consideration to you or your child, and without additional authorization, in future video or photographic projects that shall remain the sole property of Snow Valley Ski Resort.

Group Friend Requests
If you would like to request for a camper to be placed in the same group as another camper(s) it must be included on all campers' registration forms. While we will do our best to accomodate group requests, please be aware that groups are created based on age and developmental levels as well as required supervision ratios. For reference, our age groups, Explorerers and Adventurers, are split further into specific ages. For instance, Exploreres will be divided into a group of 4 year olds, another group of 5 year olds etc.
