Please use this form to request a refund of a team or candidate fee. This will allow us to properly track and process your request and ensure your funds are returned to you as soon as possible.

NOTE: If you have more than one refund to request you will need to submit this for each request due to accounting reasons. For example if you are a team member and also sponsored someone, you would sumbit 2 forms, one for the team fee refund and then one for the candidate fee refund.
