I understand that my child needs to be completely potty-trained before the first day of school. I also understand that I am responsible for the following forms that must be completely filled out, signed, dated, and on file at Hope Lutheran Preschool by my child’s first day of school:

- ODJFS Form 01234
- ODJFS Form 01305
- Immunization Record
- Photo-Video Permission Form
- Guardian Permission Form
- Potty-Training Policy & Acknowledgement
- ASQ Packet

Your signature below indicates that you understand and agree to the conditions of this Registration Form. You also agree that you are responsible for monthly tuition payments due the first day of the month, (September thru May) unless you have officially withdrawn your child from this program by completing an official withdrawal form from Hope Lutheran Preschool. If the withdrawal form is received after the first day of the month, tuition will not be refunded for the month of the withdrawal.
