I, the undersigned, have read and agree to the Club Rules and Regulations below. I also agree to adhere to the Harassment Policy (on the website at http://www.perthtaytennis.ca/resources/Harassment-Policy.pdf) and the Concussion Prevention and Management Policy (on the website at http://www.perthtaytennis.ca/resources/Concussion-Prevention-and-Management-Policy.pdf), as conditions of participation as set out by the Perth Tay Tennis Club (the Club) and waive any liability towards the Club with regard to any illness, loss or injury incurred as a result of playing in a Club organized event whether on the Town of Perth Conlon Farm Recreation Complex or at other locations.
I understand that league play may be suspended due to the evolving COVID-19 situation.

PERTH TAY TENNIS CLUB INCORPORATED

The objectives of the Perth Tay Tennis Club Inc. (PTTC) are to provide recreation for members of the community of the Town of Perth and surrounding municipalities, to promote and develop the game of tennis at both junior and adult levels.

Opportunities to play tennis on both a social and a competitive level are offered. The highest level of sportsmanship is expected from every player. In any situation not covered by on-court rules and tennis etiquette, the OTA Code of Conduct and Tennis Canada Rules of the Court shall apply. If the activities of a member violate the objectives and/or are not in the interest of the PTTC (including conduct in violation of the Harassment Policy and conduct contrary to rules and regulations of the Town of Perth in relation to the use and enjoyment of the Conlon Farm Recreation Area), membership may be terminated or application for renewal refused.

DECLARATION OF COMPLIANCE – COVID-19

By signing this membership form I agree to self-assess before coming to play tennis and if I am feeling unwell or have tested positive for COVID-19 and am within five days of the start of symptoms, will not go to the courts.  If I am scheduled for league play, I will find a substitute. I will contact the league convenor at my earliest opportunity if I am unable to find a substitute.

CLUB RULES AND REGULATIONS

Only club members may participate in club events such as leagues, tournaments and drop-ins, including spares, except as per the guest policy below.
The club does not rent court time for scheduled Drop-Ins. When you are playing on a court that has not been reserved, all 4 courts are in use and a group is waiting to play, please observe etiquette of longest playing group yielding their court to group that has waited for ½ hour.
Proper tennis attire and footwear must be worn on the courts.
All organized activities must be cleared through the Board of Directors.

GUEST POLICY

Guests are welcome to play in organized club events (leagues, drop-ins and tournaments) for a fee of $10 (20% of which is paid to the town). The balance of the fee may be applied to a new membership in the same season. This policy may be rescinded or amended at any time without prior notice, at the discretion of the Board of Directors.
