Eyelash Lift/Lift and Tint Consent

I agree to have an eyelash lift/ lift and tint applied to my natural eyelashes by the licensed professional.

I understand that in rare occasions there are risks associated with having a lift/lift and tint service performed. I further understand that in rare cases as a part of the procedure eye irritation or discomfort could occur.

I understand and agree to the aftercare instructions provided by the licensed professional for the use and care for my lash lift/lift and tint. I realize and accept the consequences of failure to adhere to these instructions may cause the results to not take or decrease the time the lift/lift and tint will last.

I am informing the licensed professional of the following

Current use of contact lenses, which I may be asked to remove during the procedureCurrent use of anything containing oil such as sunscreen or moisturizer around the eyesCurrent use of eye dropsCurrent allergies or sensitivitiesHistory of reoccurring eye infectionsHistory of dry eyesRecent use of chemotherapyOther medical conditions which would prohibit or compromise the results and longevity of the lift/lift and tint

I agree to the following eyelash lift/lift and tint follow up and maintenance instructions

No mascaraNo oil based products around the eyesNo water can come into contact with the eyes for 24-48 hours after procedureA follow up appointment will be required every 6-8 weeks to maintain results

I am consenting for photos of my face and eyelashes to be take to be used for social media or promotions

This agreement wil remain in efect for this procedure and all follow up appointments conducted by a licensed professional. I read English and understand that this consent is legal and biding.I have read and fully understand all information in this agreement. I am over 18 years of age and consent to the agreement and to the eyelash lift/lift and tint procedure
