    Authorization / Consent / Waiver:

    • I understand that the player may be involved in a number of activities as part of the program. These activities may include, but are not limited to, walking, running, and other activities. While all programs are supervised by staff who instruct players in safety, the player may still get injured, or the player's property may be damaged, as a result of participating in the program. By knowing and understanding the program activities and risks, I freely agree, or allow the player, to participate in the program.

    • I authorize the Academy or its staff or its agents to seek medical attention and/or admit me, or the player, to hospital; In the event that the player is injured, ill or in need of medical attention.

    • I consent to the publication of my, or of the player's, name, photo, or video on the website, newsletter, blogs, social media, or by the news media.

    • I agree to indemnify the Academy, its staff, and members, against any loss from any claim hereafter made against it or them or any of them by or on behalf of the player and arising directly or indirectly from such participation, including transportation.

     

    Cancellation / Refund Policy:

    • The program subscription may be cancelled at anytime by sending a request for cancellation to info@fctigersacademy.ca. Please allow 5 business days for processing.

    • Any deposit(s) made to use the sporting good(s) or kit(s) will be refunded upon receipt of the good(s) or kit(s) in reusable condition.

    Declaration:

    • I understand that by providing false information or misrepresentation may result in the expulsion of the player from the program.
