I understand that this protocol is being temporarily modified to use virtual means to interact with research participants. I will exercise all due caution and will not collect any information I was not already approved to collect. I also attest that I will not restart any in-person participant contact until the pandemic emergency has been lifted and I have been told by the IRB that I can proceed. I understand that failure to follow this requirement will result in a protocol deviation and/or protocol violation.
