I have completed a Fitness Quest screening form and can confirm it is still correct and valid for participation during any online classes I attend
I agree that it is my responsibility to ensure my workout environment is clear of any hazards including pets and other household members
I can confirm that I am healthy and physically fit to exercise and agree to inform Fitness Quest of any changes to my health status
I understand that it is not permitted to distribute or record the virtual classes for later use and that no other person is permitted to take part via my booking
I can confirm that the above questions have been answered to the best of my knowledge and agree to contact Fitness Quest with any change of circumstance.

I agree to take full responsibility for all physical activity completed whilst participating in Fitness Quest virtual classes.
