Release of Liability

I understand that Can Do Canines trains and places assistance dogs and that I may be in contact with a dog or dogs as a client or while working, volunteering, training, or visiting the Can Do Canines facility, event, or activity. I agree to follow all verbal and written guidelines/instructions concerning general and safety practices pertaining to the dogs given to me by Can Do Canines staff.

In consideration of the opportunity to participate in Can Do Canines activities, I hereby assume all risks of injury, illness, death or other loss arising from or relating to my participation in Can Do Canines activities. I hereby release, agree not to sue, and forever discharge Can Do Canines and its affiliates of and from any liability, claims, causes of action, and claims for damages I have or might have against them arising out of or in connection with my participation in activities with and involving Can Do Canines.

This release shall be binding upon my heirs, administrators, executors, and assigns. I represent that I have read and understood this Release of Liability and acknowledge that this release is being relied on by Can Do Canines in permitting me to participate in Can Do Canines activities.

I acknowledge and accept all the terms outlined in this waiver.

Photo/Media Release

I grant to Can Do Canines the right to use, edit, disclose, reproduce, assign and/or distribute for advertising, marketing and publicity purposes, my, and my minor child’s name, image, likeness, and description of my, my minor child’s activities with Can Do Canines. This information may be disclosed to various media outlets and/or Internet publications.

I agree to release Can Do Canines from any liability including any claims of invasion of privacy resulting from the use or disclosure of information in accordance with this authorization. I agree that I shall not be entitled to any payment as a result of Can Do Canines’ use or disclosure of such information. I understand and agree to the terms of this authorization.

I acknowledge and accept all the terms outlined in this waiver.

Volunteer Confidentiality Agreement

As a protective measure of the privacy of clients/graduates contracting with Can Do Canines, we ask that each volunteer read and sign this confidentiality agreement.

• I understand that in the course of my volunteer work with Can Do Canines, I may be exposed to information of a confidential nature pertaining to staff/clients/graduates/dogs in training and their families.
  
  
• I will consider as confidential all information which I may hear, see or read directly or indirectly, and will not seek information in regard to a client/graduate/dog except as it pertains to my work as a volunteer for Can Do Canines.
  
  
• I will uphold the standards of Can Do Canines and will safeguard its reputation by maintaining the highest standard of confidentiality.
  
  
• I understand that any breach or compromise of confidentiality disclosed by me may result in immediate dismissal from Can Do Canines and could result in disciplinary or legal action.
  
  
• In the event that any breach or compromise of confidentiality the volunteer agreement can be terminated immediately at the discretion of Can Do Canines. Can Do Canines will provide no legal representation on my behalf.
