I understand that applying does not ensure approval and that untruthful answers or failure to comply can result in the forfeiture of any Happily Furever After Rescue animal fostered by me.

I certify that the above information is accurate, and I understand that this will be verified through reference and/or vet checks.

I agree that I will not foster for another organization while volunteering with Happily Furever After Rescue.

All supplies and food are provided, however I understand that any additional puchases, including travel to and from the vet for appointments will not be reimbursed.

I understand that Happily Furever After Rescue cannot guarantee the health and condition the animal will arrive in. As a foster, I will provide the proper care and adminster medications and preventatives as needed.

I agree that if I'm unable to foster anymore, I will return the animal and supplies to Happily Furever After Rescue and provide enough time for the organization to secure another foster placement.

I understand that all animals are under legal ownership of Happily Furever After Rescue and can be removed from my home at any time. The animal(s) must be returned upon completion of the foster period, unless other arrangements have been made with Happily Furever After Rescue.
