OVER THE COUNTER MEDICATIONSThe following over the counter medications are available at Lakeland Christian Academy. Please select each individual medication you are giving permission for your child to receive. All medications will be given in accordance with the manufacturer’s label directions.
If you choose to refrain your child from receiving any OTC meds, please select that option and move onto the signature line below.
I give permission for this information to be shared with the school staff and emergency medical staff that might have direct contact with my child. I also give permission for school staff to care for and to meet the immediate health needs of my child.
I authorize the Lakeland Christian Academy staff members to administer the medication described below to my child. Per Indiana Code, the guidelines for administering medication at schools are as follows:
Prescription Medications:1. No prescription medication will be given unless the school has on file a medication authorization form, completed and signed by the doctor, the medication is in the original container with the original pharmacy label and the child’s name. *Controlled substances must be counted by both staff and person dropping off/picking up, and #/amount signed/dated in agreement.2. The authorization to administer medication from the child’s parent/guardian must be on file at the school.
Herbals:1. No herbal or vitamin supplements will be given unless the school has on file a letter/doctors note from the doctor prescribing. All herbal or vitamin supplements must be sent in the original container labeled with the student’s name.2. The parent authorization to administer medication from the child’s parent/guardian must be on file at the school.
Parent/guardian accepts the following:- Parent/guardian accepts legal responsibility for the safe arrival of the student’s medication to his/her school. It is the responsibility of the parent/guardian to maintain a supply of the medication for the student.- The school nurse may contact the student’s physician with questions regarding the giving of this medication.- Medication will be given during school hours only unless prior arrangements have been made with the school nurse.- Medication may be dispensed to your student by a UAP (Unlicensed Assistive Personnel) which may include, but not limited to principals, secretary, teachers, health room aide, bus driver, or other employees. All UAP’s will be trained yearly according to IC 34-30-14-2.- ALL Medication needs to be turned into the school nurse on arrival at school. All students in PK-8 grade must have a parent/guardian or other designated person who is over 18 years of age drop of or pick up medication.- Any unused medication that is not picked up by the last day of school will be discarded.

This is pursuant to my right to refuse vaccination on the grounds that vaccinations conflict with my religious beliefs. Pursuant to Indiana statute, I am providing a copy of this statement to our child’s school administrator or operator of the group program.
As a parent/guardian of this student, I hereby consent to the use of photographs/videotape taken during the course of the school year for publicity, promotional and/or educational purposes (including publications, presentations or broadcast via newspaper, internet, or other media sources). I do this with full knowledge and consent and waive all claims for compensation for use or damages.
In consideration of Lakeland Christian Academy field trips for the school year we hereby release and forever discharge Lakeland Christian Academy of the Winona Lake and Warsaw, Indiana campuses, the employees of Lakeland Christian Academy, and the Board of Directors of Lakeland Christian Academy, from any actions, causes of actions, claims, and demands that result from any damage, loss, or injury, which hereafter may be sustained by our child in consequence of participation in said trips and all activities engaged in during said trips.
This extends and applies to, and also covers and includes, all unknown, unforeseen, unanticipated and unsuspected injuries, damages, loss and liability, and consequences thereof. The provisions of any state, federal, local or territorial law or statute providing in substance that release shall not extend to claims, demands, injuries or damages which are unknown or unsuspected to exist at the time, to the person executing such release, are hereby expressly waved.
