Please review the list of documents you will need to complete your application. You cannot save your application and return to it later. So be sure to have your documentation when you are ready to complete and submit your application.
If you are not a current or former client of SSCAC, you will need to provide documentation so we can verify your income. Documents can include:
4 most recent pay stubs for all employed household members
2019 tax return
most recent bank statement(s)
recent eligibility letter/benefit statement for SNAP/WIC, Section 8, Unemployment, etc.
If you are applying for rent assistance, you will need to upload a copy of your current lease and provide your landlord's contact information.
If you are applying for mortgage assistance, you will need to upload a copy of your most recent mortgage statement with payment voucher and your lenders contact information.
If you are applying for utility assistance, you will need to provide a copy of your most recent utility bill with payment voucher and contact information for your utility company.
If you are applying for another type of assistance, you will need to provide a copy of the vendor invoice, bill, or quote and contact information for the vendor.
