Please calculate your entry/entries and send payment using "Friends and Family" to damnyankeespsaclub@gmail.com after completing this form to reserve your spot. If the payment is not sent "Friends and Family" it will be sent back. Please note that entries will not be reserved until payment is received and a confirmation email has been sent verifying your entry is complete. In keeping with PSA rules, only 25 teams per day will be accepted. If a level 3 competitor signs up, the limit drops to 20 teams per day. All level 3 competitors MUST pre-register a minimum of 4 weeks in advance. If we receive more than 20 entries prior to the 4 weeks, then there will be no spot open for a level 3 entry. Refunds are at the discretion of Damn Yankees PSA Club.

Release:  It is the understanding that the above handler/canine team are entering this trial AT THEIR OWN RISK.  I understand that serious accidents can and do occur during dog training, competitions and other dog related events activities.  I am assuming all risk liabilities and responsibilities and will hold harmless PSA K9, Damn Yankees PSA Club, their agents, partners, employees and volunteers from ANY AND ALL liability arising from this dog-related event and practice training.  I also agree to hold harmless any of the above mentioned from property damage, theft, death or personal injury to my dog or myself.  I also agree to have complete control over myself and my dog at all times.  By signing this entry form I acknowledge that I have read and understand the trial waiver and read and understand all PSA rules that governs trials. There will be no refunds given to any handler that has been ejected from this trial for any reason.  In addition, by signing this release I give consent to any representative of PSA K9, Damn Yankees PSA Club and Wise K9 Photography to take pictures of events and activities surrounding the events, said pictures shall be the sole and exclusive property of Wise K9 Photography and Damn Yankees PSA Club for their sole and exclusive use and distribution. Further by signing this, I release any rights to the sale, posting or distribution of any pictures taken by Wise K9 Photography and Damn Yankees PSA Club.
