I hereby authorize the veterinarian to examine, prescribe for, or treat the above-described pet.  I assume all responsibility for all charges incurred for the care of the animal.  I also understand that ALL PROFESSIONAL FEES ARE DUE AT THE TIME SERVICES ARE RENDERED.

State of Virginia Law 54.1-3806.1 requires that any animal medical care facility which does not provide continuous medical care provide the client or agent a disclosure form which states the days and times when continuous medical care is not available at the facility.  This disclosure form must be signed prior to rendering treatment and will be kept on file by Springfield Animal Hospital.

Springfield Animal Hospital provides medical care during the following hours:

Monday through Friday: 7:00 AM - 7:00 PM

Saturday: 8:00 AM - 2:00 PM

Holidays that we are closed: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas.  *Should the holiday fall on a Sunday, we will be closed the Saturday before.

Springfield Animal Hospital does NOT provide medical care during the following hours:

• Overnight, from closing time of 7:00 PM to opening time at 7:00 AM or 8:00 AM the next morning as described above.
• On weekends, from closing time on Saturday at 2:00 PM to opening time on Monday at 7:00 AM
• On holidays, from closing time on the day before the holiday at 7:00 PM to opening time on the day after the holiday at 7:00 AM or 8:00 AM as described above.
• Should the holiday fall on a Sunday, from closing time on Friday at 7:00 PM to opening time on Monday at 7:00 AM
• Should the holiday fall on a Monday, from closing time on Saturday at 2:00 PM to opening time on Tuesday at 7:00 AM

***Pet Care Attendants are on site to care for boarding guests during limited hours when the hospital is closed for business***

In some instances, you and your pet may be referred to an emergency clinic for continued care.
