  • Privacy – Transformation Academy collects and stores information from this form to:

    ·         manage the business of Transformation Academy (including internal reporting, administrative processes and selection of scholarship and prize winners)

    ·         comply with the requirements of the Education and Training Act 2020 and other legislation[[1]] relating to maintenance of records

    ·         supply information to government agencies and other organisations as set out below.

     

    In signing this enrolment form you authorise the disclosure of your personal information on the understanding that Transformation Academy will comply with the legal requirements in relation to the use and disclosure of personal information, as set out in the Privacy Act 1993 (shortly to be replaced by the Privacy Act 2020), the Education and Training Act 2020 and other relevant legislation.  You are entitled to see any information that holds about you and request to correct any errors in that information.  To do so, contact the Enrolments Officer.

    NB:  The Privacy Act came into force on 1 July 1993 with the stated aim of protecting the privacy of individuals.  The Privacy Act requires to collect, hold, handle, use and disclose personal information in accordance with the twelve information privacy principles in the Act. http://legislation.govt.nz/act/public/1993/0028/latest/DLM296639.html?src=qs.  From 1 December 2020, the Privacy Act 1993 will be replaced by the Privacy Act 2020, which contains the same privacy principles and governs the collection, use, storage and disclosure of personal information.

     Supply of information to government agencies and other organisations

    Transformation Academy supplies data collected on this form to government agencies, including:

    ·         the Ministry of Education

    ·         Education New Zealand

    ·         the New Zealand Qualifications Authority

    ·         the Tertiary Education Commission

    ·         the Ministry of Social Development (in relation to student loans and allowances) and Inland Revenue (student loans)

    ·         Immigration New Zealand (for those who are not New Zealand citizens or permanent residents) and the Ministry of Business, Innovation and Employment 

    ·         agencies which support particular students through scholarships, payment of fees or other awards (if you are a recipient of one of these awards).

    Those agencies use the data collected from Transformation Academy to:

    ·         administer the tertiary education system, including allocating funding and the administration of the Fees-Free and Fees-Free Trades Training initiatives

    ·         develop policy advice for government

    ·         conduct statistical analysis and research.

     

    Your personal details (name, date of birth and residency) as entered on this form will be included in the National Student Index and may be used in an authorised information matching programme with the New Zealand Birth Register.

     

    The government agencies above may supply data collected on this form to Statistics New Zealand for the purposes of integrating data with data collected by other government agencies, subject to the provisions of the Statistics Act 1975.  Integrated data is used for the production of official statistics, to inform policy advice to government and for research purposes.

    In handling data supplied by you on this form, the government agencies are required to comply with the provisions of the Privacy Act 1993 (and after 1 December 2020, the Privacy Act 2020).

    When required by law, Transformation releases information to government agencies such as the New Zealand Police, Department of Justice, Ministry of Social Development, and the Accident Compensation Corporation (ACC).

    Information collected on this form may be supplied to other educational organisations for the purpose of verifying academic records.

    Fees – In signing this enrolment form you undertake to pay all fees as they become due, and to meet any late fees and collection charges associated with debt recovery. Transformation Academy’s policy on withdrawal and refund of fees may be obtained from the Enrolments Officer.

    Rules – In signing this enrolment form you undertake to comply with the published rules and policies of Transformation Academy with regard to attendance, academic integrity and progress, conduct and use of information systems.

    Declaration – I declare that to the best of my knowledge all the information supplied on, and with, this enrolment form is true and complete, I agree to abide by the conditions described above, and I consent to the disclosure of personal information as described above.

  • Domestic Student Qualification

    To qualify as a domestic student, and so be entitled to the Government tuition subsidy, you must be:

    ·         a citizen of New Zealand (including students from the Cook Islands, Tokelau, or Niue who have New Zealand citizenship) or

    ·         a permanent resident of New Zealand or

    ·         a citizen or permanent resident of Australia residing in New Zealand or

    ·         a citizen of New Zealand (including students from the Cook Islands, Tokelau, or Niue who have New Zealand citizenship).

    If you are studying overseas full-time and face-to-face at a campus or delivery site of a New Zealand tertiary education organisation (TEO), you may also qualify as a domestic student if

    ·         you are a permanent resident of New Zealand or a citizen or permanent resident of Australia and

    ·         you are studying in an approved country and

    ·         the overseas study is Level 7 or above on the New Zealand Qualification Framework.

    The 29 approved countries are in the Asia, Latin America and Middle East regions. A full list is available on the Education New Zealand website.

     

    You must provide evidence of citizenship or permanent residency. To do so you must produce one of the following:

    •   Birth certificate with place of birth stated as New Zealand, Cook Islands, Tokelau, or Niue.
    •   New Zealand passport.
    •   A certificate of identity.
    •   A statement of Whakapapa, including date of birth, countersigned by a kaumatua.
    •   A New Zealand certificate of citizenship.
    •   Overseas passport with residency stamp.

    You can bring the original documentation to the enrolment desk, alternatively please provide a certified copy. This means a photocopy, photograph or scanned copy of your original document, signed as being a true and accurate copy by a Justice of the Peace (JP), Barrister or Solicitor, Notary Public, Court Register or Deputy Registrar, Member of Parliament, Land Transport New Zealand, Public trust, or local authority employee designated for this purpose.  When a learner is in a remote community and unable to access a person listed in the Oaths and Declarations Act, a member of the New Zealand Police, school principal, minister of religion, or general practitioner is acceptable.

    International students must bring their passport with them when they enrol.

    Please note that your name, date of birth and residency as entered on this enrolment form will be included in the National Student Index, and will be used in an Authorised Information Matching programmes with official governement information such as the New Zealand Birth Register, to verify the information provided. 

     

    On occasion, more than one National Student Number is created for an individual. The Ministry regularly monitors the quality of the National Student Index and, when duplicates or errors are discovered,  it works with providers and government agencies to merge duplicates and correct errors. This may require the documentation you provide at enrolment being shared between agencies authorised to access the National Student Index to make these corrections.

    • I agree to abide by the statutes, regulations and policies of Transformation Academy and understand that any breaches of these will result in expulsion. I am aware that this agreement is subject to the laws of New Zealand.

      personally completed the form. I have not withheld information which could have a bearing on my enrolment or the conditions of my
      enrolment. I acknowledge that Transformation Academy may suspend or terminate my enrolment if false information has been supplied or

    • I understand that all documents submitted with this application become the property of Transformation Academy and will not be returned to
      applicants. I agree to supply further documentation requested by Transformation Academy for the purpose of my enrolment and I authorise Transformation Academy to release information regarding my application to Immigration New Zealand (INZ) and the Department of Labour where Transformation Academy considers the information relevant to my immigration status.
    • I authorise any agency holding the source of any information I have provided on this form to release that information Transformation
      Academy upon request.
    • I acknowledge receipt of the Student Handbook which includes the conditions of enrolment, Code of Practice and the Withdrawal and Refunds Policy. I have read, understood and agreed to these conditions.
    • In signing this enrolment form you undertake to pay all fees as they become due, and to meet any late fees and collection charges associated with debt recovery. Transformation Academy’s policy on withdrawal and refund of fees is published in the Student Handbook.
    • I allow my/ my student's work, photos, videos, recordings to be used by Transformation Academy for various documents and media releases, both private and public. The Academy will still endeavor to seek permission from the student or their caregiver/guardian once more before such actions take place.

  • Responsible Use of ICT Agreement 

    Please refer to the Enrol section of our website for our Digital Citizenship Policy before reading this document and signing the Student and Parent Declarations overleaf.
    When using information & communications technologies (ICT) at Transformation Academy I will always be a good digital citizen. This means that:
     
    I will be a confident and capable user of ICT.
    I know what I do and do not understand about the technologies that I use. I will get help where I need it.
     
    I will use ICT for learning as well as other activities.
    I understand that technology can help me to learn. I also know it can also be used to talk to people, to buy and sell things and to have my opinion heard. I know when and where it is OK to do each one.
     
    I will think carefully about whether the information I see online is true.
    I know that it is easy to put information online. This means that what I see is not always right. I will always check to make sure information is real before I use it.
     
    I will be able to speak the language of digital technologies.
    When people talk online the things they say can be quite different from a conversation they might have if they were sitting next to each other. I know that I must try to understand what people are saying before I react to them. If I am not sure, I can ask them or someone else to explain.
     
    I understand that I may experience problems when I use technology but that I will learn to deal with them.
    I understand that there will be times when technology may not work as I expected it to, or that people may be unkind to me online. I know that there are ways I can deal with these things. I also know there are people I can go to, to get help.
     
    I will always use ICT to communicate with others in positive, meaningful ways.
    I will always talk politely and with respect to people online. I know that it is possible to bully or hurt people with what I say and do on the internet. I will think about the effect that my actions have on other people.
     
    I will be honest and fair in all of my actions using ICT.
    I will never do anything online that I know will hurt anyone. I will make sure what I do is not against the law. I will ensure my actions don’t break the rules of the websites that I use. When I am not sure about what I am doing I will ask for help.
     
    I will always respect people’s privacy and freedom of speech online.
    I understand that some information is private. I will be careful when using full names, birthdays, addresses and photos of other people and of my own. I also know that I will not always agree with what people say online but that does not mean that I can stop them or use it as an excuse to be unkind to them.
     
    I will help others to become better digital citizens.
    Being a good digital citizen is something we all have to work at. If I know that my friends are having problems online, I will try to help them. If I see that someone is being unfairly treated online then I will speak up rather than just watch it happen.
     
    I will respect all lCT systems in use at Transformation Academy and treat all ICT equipment/devices with care.

  • Student Declaration  

    I understand that these guidelines for responsible use are to be followed when using any technology whilst at Academy or whilst on any Academy activity. I am aware that this may include the use of a device that the Academy does not own.
     
    I understand that if I breach this responsible use agreement, I may lose access to Academy ICT services including the use of the internet, on Academy owned devices or any personally owned device used at the Academy.


     
    Parent/Guardian Declaration

    I understand that Transformation Academy is supporting students as they learn the skills required to become successful digital citizens.
     
    I understand that our Academy has a policy and associated procedures which outlines the digital citizenship approach and how this supports teaching and learning. This policy is available in the student handbook.
     
    I understand that the Academy provides access to the internet and other communications technologies because it believes that they enhance the teaching and learning process.
     
    I understand that the Academy encourages responsible behaviour by students when using technology to help protect themselves and each other from experiencing harm online. I am aware that this “Responsible Use Agreement” is part of that, and that it encourages students to consider how their actions can affect themselves and those around them.
     
    I have read the student declaration and have talked with my student about what it means to them. I am happy that my student understands what this means, and that they are capable of working within the guidelines.
     
    I am aware that students can experience challenges when using technology, but that the Academy makes every effort to support students to manage those challenges effectively. I understand that by learning to deal with them in a safe environment with the appropriate support they will be better prepared to deal with those challenges in the future.
     
    If I have questions or concerns about the way in which technology is being used by my student at Academy, I know the Academy is happy to discuss this with me, and I am aware that I am welcome to do this at any time.
     

  • Consent to Transport Waiver

    I give my consent for the student identified to be transported by Transformation Academy and will assume all liability for any injury that may result during the transport or at the Transformation Academy
    Further, by signing:

    1. I will not hold Transformation Academy, its staff or anyone acting on its behalf, responsible or liable for injury occurring to the named person in the course of travel.
    2. I hereby accept financial responsibility for personal items lost by the person identified herein.
    3. I authorize Transformation Academy to transport and to obtain, through a physician of its own choice, any emergency medical care that may become reasonably necessary for the person in the course of such travel, and agree to accept the cost of the transportation and/or treatment by medical personnel or facility.
    4. I accept full responsibility and hereby grant permission for me or my minor child to travel with Transformation Academy.
    5. I consent to Transformation Academy’s use of staff member’s private vehicles, if necessary.
       

    This Waiver and Release will be valid for all transportation occurring as of and after the date below.
