Friendly Note to Applicant: Our application is designed with one purpose; to protect the companion animal that has come into our care. We require completed application, home inspection & family interview. Please be sure to fill it out in detail and not skip anything. All answers are strictly confidential used by us to ensure a good match for our beloved rescued pets.

We strive to make sure we match each of our dogs with the best suited home possible. If you don't have a particular pet in mind that's okay, We are matchmakers committed to helping our applicants and our dogs find the perfect match. We will consider your information "match" you....Of course if there is no "chemistry" between you and the pet/s we suggest you are under no obligaiton to adopt...

The first step to getting approved is subbmitting a completed application, & a copy of lease, HOA agreement, or proof of home-ownership, we want to make sure that pets allowed where you live, We do not want our pets "snuck" into a home where when "landlord catches you" you are given the boot! We do not want you or our beloved rescued pets homeless! We also request when submitting an adoption application kindly make a donation to help us keep the website, online form service and dog rescue going... Every penny donated goes to the dogs, we are volunteers. Donations can be made here

Next, Please give us time to do a thourough application fact verification, veterinarian & personal reference check, we will then setup and do a home visit to meet your family, and make sure things are safe for a dog... When all of the above is an order a meet and greet with the dog & potential adoption appointment will be set up for you in Southern VT.

Proof of home ownership and / or copy of lease and or HOA pet policy should be attatched to this form (where it says upload).. Applications submitted either Incomplete, without proof of homeownership/ lease will be rejected. Please be sure you do not skip anything,on form if an answer does not apply type NA (not applicable).

A successful adoption depends on selecting the right dog for your household and a good understanding of the dog's needs and care requirements. Please help us with this process by answering the following questions honestly & completely to the best of your ability. Let's Begin!

So we can best match our pets to the most well-suited homes we require applicants list every adult, child and Pet that lives in this household, including both full-time & part-time occupants.

Therefore, failure of applicants to list every home occupant, or omit or falsify info on this application will result in a rejected application, or an annulled adoption if the discovery of omission or falsification is made post adoption, accordingly, if adoption is annulled, then any adopted animal will be siezed by Rescue, without refund.
